In Episode 5, Nine Perfect Strangers opens with flashbacks. At Tranquillum, Masha lost one of her residents. CPR pictures, flatlining heart monitors, and blinding visuals all confirm this. Masha is haunted by ghosts from her past, as is Zoe, who has visions of Zack standing in front of her.
She discovered that she and Zack were close, and she misses him terribly. Zoe acknowledges that she is extremely lonely, and that the loss of her son has left a significant hole in her life.
In Episode 6, Francis continues to be plagued by ghosts from the past in Nine Perfect Strangers. Paul, in particular, appears as a teeny little guy racing across the apartment. Paul admits that she actually handed him $128,000 in his scathing outbursts.
Lars, like many others, is haunted by the ghosts of his past. Remember when he said something about his Christian upbringing? All of the youngsters congregate around him, harassing him and calling him a faggot.
In Episode 7, Carmel attacks Masha in the first episode, mistaking her for Lillian, the lady who went away with her husband. Carmel refers to Masha as Lillian as she repeatedly hits her in the face, causing blood, until Yao subdues her.
When the other guests witness Carmel being brought to the infirmary, they become concerned, wondering whether Masha overdosed them. Frances believes she sees herself without a head, and Tony begins to feel odd as well. In the middle of the pandemonium, Ben and Jessica appear to be the only ones who are calm, and they decide to repeat their wedding vows.

Masha goes to Carmel’s room to calm her after ingesting the hallucinogens with the Marconis. She discovers Carmel distressed and packing her stuff, but she attempts to persuade her to stay and finish the therapy.
Carmel keeps reiterating that she shouldn’t be near Masha and eventually looks away from the Tranquillum House leader, fidgeting with her eye. Carmel turns to face Masha again in the episode’s last moments, showing that one of her eyes is clouded and blue, and Masha gets a short flashback of being shot.

As a result, Carmel exposes herself to be the same person who shot Masha and nearly killed her years before. Masha sees in the flashback is the same person who assaulted her and has a clouded eye. Carmel also pulls a phone from a hidden location in her room, implying that this odd Tranquillum House visitor has been sending those dangerous texts to Masha the entire time.
Masha slept with her husband many years ago, presumably setting him on a road that led to him abandoning Carmel, and Carmel is likely driven by a desire for vengeance.
Despite her claims that she forgives Masha, it appears that Carmel has been preparing her vengeance for years, and has even attempted to murder Masha. She also seemed to be in control of her faculties, so when she assaulted Masha and claimed to have hallucinated that she was Lillian, she was most likely lying.
